Your First Visit

We are pleased to be your oral healthcare providers. Our goal is to provide you with first-rate dental care as we get to know you and develop a long-standing relationship with you and family. Because we value strong communication, we strive to create a comfortable and caring environment for your first visit—and every visit thereafter.

Your first appointment as a new patient will take approximately one hour. One aspect of the appointment involves gathering information about you. To do this, we will send you the new patient form for you to complete. Please feel free to complete it prior to your scheduled appointment.

Also, please remember to bring the following with you at the time of your appointment so we can include this information with your records:

• Any insurance cards that might apply to you or your family
• A list of medications that you currently take
• Any recent dental x-rays from another office

If you are unable to make your scheduled appointment, please notify us at least 24 hours in advance. We can reschedule the appointment for a more convenient time, if necessary.
